There is an acknowledged concern that some trainees in general surgery fear to choose endocrine surgery as a subspecialty because of a perceived lack of training opportunities and concerns about employability at Consultant level within a general surgical directorate.
Similar to most other surgical sub-specialities, completion of a post-CCT fellowship is increasingly becoming the only possibility for gaining confidence in operative skills and decision making as Consultant Surgeon with expertise in a surgical subspecialty. Travelling abroad for gaining such experience is less popular compared with previous decades and many trainees focus on finding regional/national positions. In this context, the BAETS is keen to increase the visibility of all training opportunities available within the UK. Despite variations depending on local expertise and interests, it is expected that such training will include:
- comprehensive exposure to the management of benign and malignant tumours of the thyroid, parathyroid
- when available, training in adrenal gland surgery should be clearly described
- regular exposure to discussion within MDTs for thyroid cancer and adrenal pathology
- educational activities that will ensure knowledge of anatomy and physiology of endocrine system and ability to diagnose endocrine surgical diseases in sporadic and familial conditions. This can be secured via regular journal clubs or involvement in clinical research project.
The Educational/Clinical Supervisors for these trainees are expected to be active members in good standing of the BAETS and to demonstrate contribution to UKRETS. The Fellows should complete training with a minimum of 50-100 endocrine procedures as the first assistant and/or as the operating surgeon. This should include central and lateral neck dissections, parathyroidectomy and redo parathyroidectomy, and minimally invasive and open adrenalectomy. In addition, Fellows should gain exposure to cervical ultrasound for the detection of thyroid disease and for USS-guided FNAs and should become proficient in assessing vocal fold function by direct and/or indirect laryngoscopy or laryngeal ultrasound. At the end of their term, Fellows should present an operative logbook to their educational/clinical supervisor(s) and should forward to BAETS a summary of the experience gained.
